14 month old baby “Justin” is in the process of being discharged from a major medical center in Boston. His home is in Willowdale, a small rural community several hours away from Boston. The baby has severe Cystic Fibrosis. There is an issue as to whether or not the baby should be sent home, as care in his home community may be insufficient. His insurance will not cover any additional time in the hospital, and his family has limited financial resources.

What would you recommend? How would you reconcile the needs of the family with the medical welfare of the baby? What would you do to advance your position and promote the welfare of the patient, while maintaining team cohesion?
