Case Study: Credentialling
The nurse midwife (or PA or NP) is denied privileges to see patients
in the hospital. There is no standard policy for this at this hospital.
The midwife challenges the medical staff's authority to deny her the
ability to care for her patients. Doctors and nurses have lined up on
both sides of this issue, and it is causing generalized conflict in
various departments.

Case Study: Scope of Practice
In this scenario the pediatric specialist (NP, PA, MD) has been asked
to provide a level of care that he/she is not educated or credentialed
to do provide (e.g., seeing adults). The specialist employer's motto
is one of patient convenience and, if you've been scheduled, doesn't
want the patient to be further inconvenienced having to visit another
practitioner. You want to stay but are concerned that you may not be
qualified to provide the necessary care.

Case Study: Productivity
Finally, there is the well known scenario about time frames -- the
organization wants providers to see a patient every 15 minutes but do
a complete job of caring for the patient and documenting to support
high level reimbursement. The provider's position is that they cannot
do a good job pushing patients through that quickly and are dissatisfied
and frustrated with their work situation. There is continued griping,
and the providers have complained to patients about this problem.
